Management of Infrarenal Aortic Aneurysm




Abstract
Thirty two cases of abdominal aortic aneurysm were treated between Ianuary 1985 and December 1995 at the Department of Surgery, Keimyung University School of Medicine. Among them only nineteen patients were managed surgically. There were 26 men
and

women of mean age 65.2 years. Most common clinical sign was abdominal or back pain in 90.6% followed by pulsatile abdominal mass in 68.8%. Mean symptom duration was 35.6 months. Associated medical diseases were hypertension in 37.5%, pulmonary
disease
in 21.9% and diabetes mellitus in 6.25%. All of them were infrarenal type and most of them showed fusiform aneurysm(27 cases). Mean diameter of the aneurysms was 6.7 cm but seemed no relation with surgical mortality. Atherosclerosis was primary
cause of
all surgically managed cases. The endoaneurysmorrhaphy in 17 cases, aneurysmectomy with aortoiliac graft in 1 case and bypass in 1 case. There was no operative mortality in surgically treated group but 5 out of 9 medically treated group were died
during
their follow up period. Among them, only one was associated with aneurysm rupture as a cause of patient death. The mortality of the other cases were caused by associated medical diseases. In conclusion, surgical management of abdominal aortic
aneurysm
should be considered not only by the ize of an aneurysm or operative mortality but also the associated medical diseases.
